Patient Information on ASD /ADHD Referrals
The demand for Autistic Spectrum Disorder (ASD) and Attention Deficit (Hyperactivity) Disorder (ADHD or ADD) assessments and treatment is more than the available appointments despite efforts of the local NHS to improve this. We appreciate that as a result patients and parents seek assessments outside our usual local referral pathways. We hope that this leaflet helps you understand the difference between providers and the implications for care. If there are aspects that you do not understand or that you would like additional information about then please let us know.
Referral
If you would like to be referred then please complete the questionnaire and if the referral is for adult ADHD, a self-assessment score.
If you know where you would like to be referred then please let us know on the form.
Prescriptions
We will not take over prescribing from either Right to Choose or private providers. If prescriptions are recommended then the Right to Choose or private providers must be prepared to continue prescribing and monitoring. You should confirm that they are prepared and able to do this as part of making your choice of provider.

Mounjaro (Tirzepatide) Update
Dear Patient,
We are aware of the upcoming changes to the availability of Mounjaro (Tirzepatide) to a very limited number of patients from 23rd June 2025.
At present, we are awaiting further guidance and arrangements from the North East and North Cumbria ICB.
Please be aware that the criteria for eligibility remain very limited.
The practice will contact you directly if you are eligible.
We kindly ask that you do not book an appointment to request Mounjaro, as we are unable and not allowed to offer it outside of the strict criteria in place.
